PHOENIX (3TV/CBS 5) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for March. 20, 2024:

- Live roach on prep counter
- Cooked rice and salsa not date marked
Civana Carefree Resort - 37220 N Mule Train Road, Carefree
3 violations
- Cubed ice from unapproved vendor
- Raw salmon stored above ready to eat mushrooms
- Cheese and eggs out of temperature
Barro’s Pizza - 13971 N 59th Ave, Glendale
3 violations
- Cardboard box of food on top of cooked chicken wings
- Dishwasher not cleaning properly
- Sliced deli meat out of temperature
